Under either level of care in the Simple Dental Smile Protection Plan, the patient may be of any age above 4 years old. No other coupons, cash discounts, or offers may be combines with this plan. Periodontal cleaning beyond two (allowed in the plan) are discounted per the plan, and will be discussed with patient prior to treatment. Patient with a Dental Benefit Plan are not eligible to participate. Plan expires one year from the date of purchase. Non refundable/non transferable.
